The user had to notice, and then investigate, their extremely high heart rate and lack of fitness, to have any reason to run an ECG. The watch didn't alert them in the conventional sense, it recorded the information and provided an additional tool to investigate the problem.
There are two mechanisms on the Apple Watch that did not trigger:
- high heart rate notification
- irregular rhythm notification
